Health & Care Foundation (formerly known as Polio Foundation) was founded in 1987 by a handful of dedicated Surgeons and like-minded persons to make the polio-affected persons stand on their two feet. Slowly the trust started working in other health care activities for the needy people. In due course, to cope up with the number of patients having free treatments, Ahmedabad Urban Development Authority donated a land measuring 5500 sq yards on which the present Hospital/Institute was built to accommodate a variety of services from Pediatrics and Pathology to Opthalmology and Psychotherapy.

The government of Gujarat has declared the hospital as a District Early Intervention Center for the treatment of congenital birth defects for 0 to 18 years of children. Eminent surgeons from UK, USA, and Switzerland visit the hospital at regular intervals to perform critical surgeries for needy patients. Others that have visited and appreciated the work of the Institute include former President of India, Dr. A. P. J. Abdul Kalam and Astronaut, Ms. Sunita Williams

As of 2020, HCF arranges outreach screening camps also in rural areas to reach out to the needy people. So far more than 300 camps have been arranged in rural areas to provide screening and treatments. On average, they take care of more than 60,000 patients per year;  irrespective of their economic condition, no patient has ever left their premises without proper treatment. Over the years, the trust has been instrumental in performing more than 25,000 surgeries in congenital birth defects.
